The CCPU Health Care Fund has partnered with Guardian x Legaci to bring you Basic Term Life insurance at no cost.

All Providers enrolled in the Health Care Reimbursement Fund in 2025 will receive these benefits automatically – There is no additional application needed, and coverage begins retroactive to 1/1/2025!

Below is a snapshot of the benefits of the Term Life plan.

Basic Life
Provider Benefit CCPU Health Care Fund provides $50,000* Basic Term Life coverage for all full time providers.
Accidental Death and Dismemberment Your Basic Life coverage includes Accidental Death and Dismemberment coverage.
Guarantee Issue: The ‘guarantee’ means you are not required to answer health questions to qualify for coverage up to and including the specified amount, when you sign up for coverage during the initial enrollment period. Guarantee Issue coverage up to $50,000 per provider
Premiums Covered by the CCPU Health Care Fund if you meet eligibility requirements
Accelerated Life Benefit: A lump sum benefit is paid to you if you are diagnosed with a terminal condition, as defined by the plan. Yes, up to 50% of the death benefit
Benefit Reductions: Benefits are reduced by a certain percentage as a provider ages. Yes, $32,000 death benefit for those 65-69; $25,000 death benefit for those age 70 and above

What is a beneficiary?

A beneficiary is someone a Provider chooses to receive all or part of a death benefit.

  • There can be more than one beneficiary – and in practice, there often is.
  • A beneficiary doesn’t have to be a person – it can also be an entity such as a church, charity, or family trust
How do I designate a beneficiary?

Download this form and fill it out, and send back to the Health Care Fund. We will manage this information and in the event something happens to you, we will communicate your designations to Guardian.

What should Providers know about beneficiary designations?

In most states, if your provider names a minor beneficiary, we’ll need letters of guardianship for the property or estate of the minor to make the payout.

  • This typically requires the legal representative (e.g., surviving parent) obtain guardianship paperwork from the court.
  • Otherwise, if a claim is filed, the claim may be held for processing until the minor is of an ‘age of majority’ — 18 years old in most states.
  • Also, when the beneficiary is a minor, we cannot accept a funeral assignment (agreement allowing part or all of the proceeds to go directly to a funeral home). This may cause a financial hardship for the person who has to pay for the services.

 

If your Provider names a beneficiary who resides outside of the US:

  • The foreign bank may delay cashing a US check.
  • We may not be able to pay residents of high-risk countries sanctioned by the US.
  • Time differences may make it more difficult to locate the person or make contact.

 

The total primary beneficiary and contingent beneficiary percentages must each equal 100%.
